Posts: 2259 Apr 4 12 6:09 AM	Do NOT forget to also submit a completed VA form 21-4142 along with the DBQ!!!!!!! (per the DBQ instructions on the VA website) If you are not a VA provider, your patient needs to complete a VA Form 21-4142 for "DBQ Purposes". (This is so we can request additional information if clarification is needed.) Open Authorization and Consent to Release Information. Fill out the medical provider information Indicate "For DBQ Purposes Only" under Comments in Box #9. Save and print the form. Have your patient sign the form. Posts: 94 Apr 6 12 3:48 PM	So if I am having fibro symptoms as well as CFS do I have my PCP fill out a DBQ for both to declare the symptoms but not necessarily the actual DX in order to tie them to GWI? Will this hurt or help the overall claim? Thanks in advance.

Posts: 2259 Apr 10 12 6:15 PM	myblueyonder, I'm very interested in more specifics because these DBQs are so new. Was the C&P to determine service connection? Was it related to the DBQ condition? Done by an outside doc or VA? Did you submit the DBQ in enough time that the RO recieved it in time to cancel the C&P?

Posts: 840 Apr 21 12 9:40 PM	Typically, the VARO will order a C&P at the VAMC. For most exams, the examiner will use the DBQ to answer specific disability-related questions necessary for rating the claim/s. A veteran can obtain a DBQ in most cases to have his or her private doctor or, a VAMC doctor (independent of a VA exam) to answer the same questions. Basically an even playing field. If the non-C&P DBQ is completely filled out, and everything is consistent, there should be no need for the C&P DBQ. There's are probably still hiccups in the system so each individual's experience may vary. I, myself, had my private doc complete the DBQ and I submitted this as well as a finding of disability for SSDI to my VARO. They (perhaps a trainee or someone not with the "program") were still going to order a C&P until I went to a supervisor and got things straightened out. Ended up with a P&T.

That said, there is the art of persuasion. Don't get the doc tee'd off at you as that won't help matters. If the doc is qualified (should have no problem if PCP is an M.D.) to diagnose and conduct the DBQ then have him do it if he "agrees" to do so. There is no need to have any opinion that a diagnosis of either fibromyalgia and/or chronic fatigue syndrome is related to Gulf theater or Afghan. service. These are presumptive diseases based upon service in one of the countries or airspace of waters recognized as GWI areas. One more thing for those wanting to use DBQs. These forms are available on-line (or were). You can get the form, have the supportive doctor complete it, gather treatment reports if you have any, and submit it as a completed, or nearly completed claim from the get-go. That way they have the claim and the evidence up front. If you have a copy of your DD-214 (showing character of discharge) all the better to submit at the same time. The more work you do up front, the less you'll get frustrated later. This is my opinion, anyway.

Not sure why, particularly since one of them flatly said she would "fully support" my VA claims for service-connection. But then she wouldn't.

The problem is, if they really don't want to do this for you, and you go over their head to force them to do it, how reliable/accurate would be the DBQ that they would fill out for you. It's unfortunately a Catch-22 situation for the veteran. I suppose that if a veteran has such a provider, how well do they really care for the veteran with the illness if they say one thing and do another (then again, perhaps their management is the problem!!!). I got lucky with my PCP, but not with my specialist until his boss was informed by a high level administrative person to complete the form.

I previously supplied the FAQs for DBQs, that would answer your question, on the forum "VA disability claims (general). VHA has a policy that states the doctors at the VA do NOT have to fill out a DBQ if they don't want to, they are to tell the veteran to file with the RO. Some of these DBQ's can really long and direct the doctor to fill out additional DBQ's. Diabetes is a good example, if you have peripheral neuropathy they have to fill out the diabetes, a peripheral nerves exams. The VA doc's don't really have a lot of time so quite a few are opting out of filling them out. Private doctors usually charge a fee to fill out forms and the VA doesn't pay it. If the form is not completely completed with all of the information required a C&P exam may be ordered. If an opinion is required a C&P exam may be ordered, there are a lot of reasons that a C&P may be ordered.

The DBQ was filled out by my VA psych and submitted with the original claim for IU. They sent me for a C&P anyway.

Posts: 169 Apr 30 12 6:32 PM	My non VA specialist was not thrilled about filling out a 6 page form using up 30-45 minutes of his time. I told him that I understood that his time was valuable and offered to pay $150 which he accepted. It was amazing the change in his disposition when he realized he wasn't going to be forced in donating his time. I believe the forms were far more complete this way. I would recommend that we offer to pay whatever their office policy is for disability forms. Just my thoughts... Posts: 49 May 1 12 1:31 PM	Spouse of vet ---- I am an RN with many years experience in case management of disability cases in non military setting. These forms are wonderful. FYI - many references on diagnoses and treatment plans are obtained in the private world from the VA models. HOWEVER, reading your posts - I see there is frustration in getting the docs to fill these in and that is SO TYPICAL. They will expect to be reimbursed for their time. One way you could assist them and have a more positive and timely outcome is to offer them one blank form and a "sample" that you have filled out with the information to make it easier for them. For instance in you have diabetes and you have copies of medical records, you could demonstrate the date when you were assigned the diagnosis. Go thru the laboratory records and state the A1C results, list the diet and activity restrictions, medications etc. If you have a family member or friend who is a nurse that you can trust with your personal information you may recruit them to assist you to put the information in place. Sometimes there may be an individual in the physician's office who is helpful, friendly and may be willing to help you. That would be worth a little treat, such as flowers, cookies or Starbucks gift cards or even a basket of fresh fruit or home grown tomatoes! You may find that offices will be helpful if you play nice and schmooze a little. If your wife or S.O. has a better relationship with the office - let them request it. Good luck! Since my husband has PTSD among his list of issues, I understand that it helps to allow someone else to intervene if anger management is an issue. Remember the objective is getting what you need. Here is MY QUESTION: My husband has his case in the works. We did not have access to these forms late last summer when he applied. However, I did make certain all bases were covered within his documentation. VA has all information collected in September 2011 but no forward motion on his decision. WOULD HAVING THESE FORMS COMPLETED AND SENT IN NOW JUST CONFUSE THEM OR BE HELPFUL? I know what would work in the private world of determining disability ratings and settlements but VA is a whole new animal. Could one of you guys offer your opinion on this? Thanks!

I said I needed it for my next vVA exam, and you dont want to get on the bad side of them .Especially I>R>S>. I had the form the next day. I may submit COPD after my other stuff finalizes. I think it will be this year for sure..

Posts: 29 Jun 27 12 5:17 PM	I didnt know you could use these until after my C&P exam last week. The C&P exam for mental health was criminal in my opinion. It only lasted 10 minutes despite repeated diagnosis of depression and anxiety while on active duty form their psychiatrist. Can I do the DBQ with a private doctor even tough the C&P "exam" was already done. I fear that if I dont have a properly filled out DBQ from a competent doctor I am going to get screwed on my claim. Or does my AD medical records trump this?
