Posts: 116 07/08/09 06:58:54	There is a form to send to ASI which is Form LC-7274-2. Some providers I've used will only file the Champ Va and not the supplement. After I get the Champva Va EOB statement, I send ASI the Form LC-7274-2, Champ EOB, and my form of payment made to the provider. So far they have been pretty fast processing and sending a check back. .
