I filled out the RFC form and sent it in about a week ago, Got a call this morning from a nurse saying that she was from the ss office and was working on my case and wanted to ask me some questions about my daily activities. Funny because it was the same questions that me and the wife filled out on the form. Filed Feb 5 2010. I did the self-report using Patrick examples. I have PTSD and I am seeing chiroprater 3 times a week counseling 3 times a week. I had two carpal tunnel surgeries both wrist both elbows(cubetial) both knees done last Jan on the same day service connected 40% right ankle 10 screws and two plates in ankle with artharitis two hernia surgeries and I will be admitted to the North Chicago impatient for PTSD, I think she took some meds this morning after she talked to me.
