Posts: 55 Apr 6 10 7:13 PM	I filled out the RFC form and sent it in about a week ago, Got a call this morning from a nurse saying that she was from the ss office and was working on my case and wanted to ask me some questions about my daily activities. Funny because it was the same questions that me and the wife filled out on the form. Filed Feb 5 2010. I did the self-report using Patrick examples. I have PTSD and I am seeing chiroprater 3 times a week counseling 3 times a week. I had two carpal tunnel surgeries both wrist both elbows(cubetial) both knees done last Jan on the same day service connected 40% right ankle 10 screws and two plates in ankle with artharitis two hernia surgeries and I will be admitted to the North Chicago impatient for PTSD, I think she took some meds this morning after she talked to me.

Posts: 64 Apr 6 10 10:03 PM	Hello, I have been reading the information on the board for a while now, and the information is really helpful. I have a question about the self report. I filled out the SSDI application online, I have also been to the SSA in person interview already (last week). I did not know about the self report until after I wen to my appt. I am in the process gathering more of my medical records in support of my claim. When I turn in those records,should I turn in my self report with them? I also would like to know if I should turn in my police report for a physical/sexual assault that happened on active duty (the cause of my PTSD/Depression)? I have not worked (except for trying a part time work from home job for about 3 months, which I quit due to stress/depression) since being honorably discharged in 1999. At the time that I left the military I was obese due to injuries and the assaults and depression, but I don't have any proof of the PTSD/Depression except for me gaining weight and going to sick call often. I didn't know that I was depressed. My unit would just give me counseling statements, send me to the dietician and put me in extra PT. No one ever thought to ask if I was depressed. I would have broke down on the spot. Anyway, the lady at the SSA office said that they would try to get my claim approved back to my last day of active duty (last time I had gainful employment), my question is this: Obesity used to be listed as a medical condition (still is but different rules apply now) back in 1999, should they apply the rules that were in effect at that time to my claim? I did claim my obesity on my SSDI application because it is also one of the reasons that I am disabled/unemployed, and it did start while I was on active duty. (I went in weighing 125 lbs and I left at 205 lbs) Note: I have had my VA PCP write a detailed letter for my file and my VA psychiatrist says he will fill out the form that SSA will send to state that I am permanently disabled. Posts: 5218 Apr 6 10 10:48 PM Go Tar Heels! Closet Vols Fan	You can turn all your information into SSA and your Self-Report as well. As for your PTSD issues rather than explain you think this or that, tell the SSA people the symptoms that are happening to you. Look up PTSD online and see how you fit into the criteria for the disorder. If you feel you match the criteria explain to the SSA people how so. Patrick

I know that they olny ask for medical, but I think this helps establish a timeline for when my PTSD/Depression (overeating) started. Since I wasn't diagnosed officially until last year by VA. Posts: 5218 Apr 7 10 6:40 PM Go Tar Heels! Closet Vols Fan	I would turn in any documents that show how PTSD has affected your behavior. Then explain how your PTSD symptoms interfere with your daily functioning. If you look at the criteria for PTSD you will see an array of symptoms listed. Find those that fit you and explain to the SSA what they are doing to you. Patrick

I am an airforce vet and know a great physician who is a non attorney representative who is 100% service connected himself and is all about helping vets. Posts: 110 Jul 29 10 12:01 AM	Thank you for sharing such valuable information and for being consistent through the years to help those in need and sometimes have to answer the same questions over and over again. I (we) certainly appreciate you. God bless you. I am in the process of collecting all of my medical records so that I can file for SSDI. I understand that the VA have a liaison that helps with the filing. Do you think using the liaison is beneficial and/or will help me to get my claim processed any quicker? I was placed on the Permanent Disability Retired List from the Navy in 1988 so I am not sure if I qualify for the Wounded Warrior Program. I know that this is SSDI forum but I have a VA question as well. I am 50% sc for depressive disorder and have been for about 20 years. I have never requested an increase until recently and I filed a REV claim for an increase and was denied. The notes from my psychiatrist and psychologist outside of the VA shows that I have been consistently depressed, fatigued and have a lack of concentration consistently for the last six months. I am not sure what the VA psychiatrist and psychologist report says as I have not gotten a copy as of yet. Also, due to my condition, the longest I ever held one job is 1 year in the 22 years. I shared this information with the VA psychologist so it should be in my records. Do you think that a self-report will help in a appeal process for the VA?

Posts: 55 Aug 29 10 10:17 AM	I just got a call yesturday from the VA rep in Madrid, Spain. SSA approved with backpay to Aug 2009! I just want to thank everyone on this site! I did submit Patrick's letter. I applied under the SSA WW program. It IS faster than going the regular route. I was denied in my first try, but won the second time around. I have read at least 90% of the posts on this site, to any NEWBIES my advice is this....read EVERYTHING on this site, it's the best source of information on the net, PERIOD! I'm still waiting for the VA, I'll update as new info comes in. v/r RicoNasa>>>>

Posts: 51 Mar 5 11 8:25 AM	You an expect to be turned down, start locking for a law firm to represent you.The SSI cold body's will turn you down for any reason.The law firm will do all of the paper work that the SSI people are to lazy to do and the firm will need your VA record to present to the SSI. The cost will be paid to the firm from your back pay ( date of claim) SSI will pay them @ 25%,with a max $$ amount.Get a law firm that specializes is SSI. It can take a few trips to Doctors that work for the SSI.

Posts: 889 Mar 11 11 6:45 PM	@r johnson. Not everyone needs a lawyer the first time around or even the second time around. If the initial application and the neccesary medical documentation are provided that shows a disability under the SSA disabilty defanition then a lawyer shouldnt be needed. A lawyer may be needed for someone who is unable to do such filing with SSA for benefits. Why pay a lawyer 25% or up to a max of $6000 for something a person can do themselfs? Initial level might take 3 months for a decision and then again at the recon level. At the ALJ level then I do recommend hiring a lawyer. I also recommend that everyone should do a self report on themselfs and also use the RFC forms no matter what stage one may be at with SSA.

I was told that I couldn't draw Social Security Disability because I didn't work for several years and didn't accumulate enough credits to draw it. Now I am 100% P&T. My question is can I draw Social Security Disability using my wife's Social Security? How can I qualify to draw Social Security Disability in the position I am in?

You may be able to apply with your wife's SS, I do not know. However, if you can demonstrate that your disability began at the time you stopped working, you can qualify using the quarters you earned just prior to your disability. You won't get extra back pay, but if you become a paraplegic in 1980 but don't apply until 2010 you can use the quarters earned prior to 1980, not 2010. This applies to all disabilities, but you have to provide the records.

I went to my local SS Office, started the process, received the packet that wanted all my information. Provided the supporting documentation, filled all the paperwork out. I received a call from my local SS Office, 28 days after I started the process, informing me that I had been approved for SS Disability. Posts: 14 May 6 11 7:18 PM	Looks like I will need an attorney to do my documentation with such wow. My mind is so scrambled. They ask me to fill the SSD paperwork out, given I have 100 T&P PTSD, and I did my best. Now they want further evaluations because they are not satisfied. One federal agency is not sufficient for another. My fighting days are not over apparently. I feel like I am still not he battlefield. Posts: 170 May 9 11 10:47 AM	I filed for SSDI in March after taking early retirement in Oct. 2010. It took 2 months to recieve a decision on SSDI. The answer was positive. A diagnosis for Parkinson's from my two VA doctors was all it took along with my statement as to how it affected my daily life and work. Now if the VA would get of the stick on my 2 1/2 year old claim for same, I'd be happy.

Posts: 889 May 16 11 3:45 PM	Patrick, Will SSA collect all the medical records from both the VA and also military hospitals that one has gone to for treatment? I am a retired Marine who goes to both VA and military hospitals for treatment. I wrote myself report this time around and also added ALL locations of treatments, dates, Drs names, the reason why I was being seen and ect... I find it hard to get these reports myself. I dont know if its because of mental issues or not but allot of records to me are hard to get. My biggest problem seems to be the fact that before I retired from the Marines in 2007 (Camp Lejeune), I did the VA physical thing for benefits and even gave them a copy of my military health records. My records seems to be sent to Winston Salam, NC instead of Honolulu, Hawaii VARO. I have tried countless times to have my files sent from NC to HI but it seems almost impossible to do. All of my correspondence with the VA always comes from Winston Salam, NC and I think this adds to the problem of getting my records/files to the SSA. Questions: Will SSA collect the information of my medical files from the locations that I have provided? How do I get Winston Salam VARO release my records/files to the Honolulu, HI VARO so that my files are actually located in the state that I now live in? Thank You in advance.
