how do i fill out a va form 21-4138? i'm so confused do i just put what happened to me or what? please help me. i would really appreciate it
I've always told veterans, "This is nothing more than a blank piece of paper with a VA form number assigned to it."

I am not an expert at what the VA needs on a 4138 but I began the ones that I included in my recent (10-08), initial claim as follows:

My Primary Care Physician (Internal Medicine)is:

I have had regular appointments with PCP since June X, XXXX. He has been responsible for my general health and well care.
Find attached the Independent Medical Opinion letter attached from Dr. XXXXX XXXXXXX.
Find attached pertinent portions of my record from this doctor that reflect(s) the following:
1. This Doctor pointed out the XXXXXXXXXXXXXXX XXXXX XXXX) in the Veteran's lab work that was accomplished at XXXXXX in June '03. See the lab

The wording will depend on the issues that you are presenting and the others are correct that a VSO can and will help with what needs to be said.
This is one that was with the claim I submitted and it was well recieved when I reviewed it with a VSO rep.
In addition, I would put you full name, SSN and claim or file # in the top margin and on the top margin of every document that you include with your claim.
