All psychological and psychiatric evaluations have a 5 Axis process. Axis IV is saying to another practitioner that you have mitigating stressors that may be
influencing your diagnosis. For example:
Axis I: Major Depression, recurrent Axis IV: Psychosocial stressors: loss of job, family problems, death in the family Severity: 4-Severe Axis IV is not a singular portion of a rating, but it is a combination of all 5 Axis's that make a decision what it is. So if you have severe stressors on Axis IV, the practitioner is saying to me these stressors are influencing your Major Depression. In other words, they are making your depression worse. Patrick

Re-read my post. I said your stressors on Axis IV are those things that influence your Major depression and is making your depression worse.

Hey Patrick your slippin' you need to be more specific on severe stressors like mother-in-laws, cats (because they are evil), Obama speeches, McCain
talking about the economy, the price of GAS...the price of Diesel (which costs less to refine than gas), Oprah, and the WE channel!!!

Psychosocial stressors are not listed in Axis IV because your doing good. That is why they are identified as stressors. Stressors on Axis IV say to another
examiner that individual's Major Depression is not good and the stressors listed are not making it good.
So you will not see on Axis IV things like, happy, overjoyed, ecstatic, carefree, and whimsical. So to put it in perspective lets use this as a guide: Axis I: Major Depression, recurrent, severe. Axis II: No diagnosis Axis II or V79.01 Axis III: Medical conditions that are negatively influencing Axis I Axis IV: Psychosocial Stressors: Those things negatively influencing Axis I or II Severity 1, 2, 3, 4, 5 with (5) being Chronic Axis V: Current GAF (Again that which influences Axis I or II and should be in unison with the severity of the disorder. So a GAF of 70 would not be in keeping with the Axis I disorder described above). Highest GAF Past Year: (Should be indicated to monitor continuity of care and progression of the disorder). Patrick
