Our problem is simple, with my significant other's schedule and short patience in making calls/checking appts/getting test results with the VA, we are trying to have a form on file that says I can call to change an appt or get a test result or get the status on a prescription.... nothing major. (Naively since my mom can call the family doctor or their specialists to make/change appts, get info, I thought this would not be so intense!) At first I was told to fill out a form (Triwest form) and after a month thinking I was permitted to help my s/o on stuff, I found out today it was the wrong form. Then I was told it was a medical power of attorney form, but after leaving messages and not getting a call back I made more calls and was told there is no such form, that he has to give me power of attorney. I'm trying to help him, not take over his life, and I don't know if there is a form, what form, where to send the form. I've been transferred through the Lexington, KY VA a dozen times today. Can anyone help guide us on this?
