You need to get a VSO, take him/her copies of everything you have sent VA and tell that VSO to get hot.  Your medical records were unavailable for review for some reason and that needs to get fixed ASAP.

Decisions are made without C&Ps.  Not sure if they do it a lot, but they do. Just depends on what info the rater has.  If he/she has what they need and don't require a C&P, then they will make the decision without one. Happened...
