2 Quick questions...and some rambling ons...1st time user of the forum!
1. Can I get all of my individual "issues" noted on a VA FORM 21-0960 M-4 (Disability Benefits Form Questionnaire) from my Navy doctor via sick call NOW while I am still on Active Duty or must I wait and us a VA doctor now or after eaos? *EAOS 30June13
2. I keep hearing "service connected"....I'm assuming if it is in your medical record while on active duty as an issue (example: TMJ) that would be considered service connected. Did they want to see a particular event while on active duty that caused an injury/disease(example: fell during training exercise) or will just going to sick call and explaining your issues (frequent headaches with no mechanism of injury explained) while on active duty be enough to be "service connected"?
I've been reading on forums that medical documents must have specific words to express the relevance of the injury or else it will be denied even if the person rates it. Example: "range of motion" must be noted as an issue in file verse just having "pain" in the area. What if the Doc or Corpsman worded it years ago and it's not to the VA's liking...what are the key words? I'd rather walk away with a clean bill of health but I have some medical issues that will hold me back from seeking unlimited employment.
I'm currently being seen for all of my aches and pains to include following up on past aches and pains from years ago which are all in my record. I don't want my final physical to be a data dump of the last 24 years.
I am reading MUCH of the forums and feeling a bit overwhelmed on the process. I know if I get everything in order (proof) and supporting documentation, the process should be smooth.
