To amend a claim submit VA 21-4138 and tell VA what you would like to do or change. If you will tell us more here we may be able to give you better advice.

I submitted a claim (reopened) for coronary artery disease and ED secondary to Type II diabetes. At the same time I opened a new claim for hypertension
secondary to Agent Orange. Since the VA has turned down, for the time being, hypertension as presumptive to diabetes, I want to amend the claim to show the
hypertension as secondary to diabetes. When I was diagnosed with type II diabetes, my doctor's assessment was "adult onset diabetes and associated
hypertension". I included that record with my claim as well as a statement from my Endocrinolgist that says that the ED, CAD, and hypertension are more
likely than not secondary to diabetes. The VA has recorded numerous high blood pressure readings and the notation that I have "stage 2, hypertensive
valve disease. Also saw my VA pcp today and she said she would support my claim as well. What should I have her submit?

Here is what I would do. Write VA using VA 21-4138 at the link I provided above. Here is a sample of what I would tell them:
On XX/XX/XX I submitted a claim for service connection for Hypertension due to exposure to Agent Orange. Please change this claim to Hypertension as secondary to Diabetes Mellitus II.If you have been treated outside the VA system include VA 21-4142 for each non-VA doctor who has treated you for a condition for which service connection is being requested. Also saw my VA pcp today and she said she would support my claim as well. What should I have her submit?There is really nothing to have her submitt. Her comments are a matter of VA record and should be pulled by the regional office as the decision is being made. If however she is willing to write you a letter in support of your claim you can send that in if you like. If it is less than one year then a NOD would be in order. If it is over one year then the decision is final and can only be reopened if you submitt new and material evidence. Here again if you can state why you previous claim for CAD was denied we may be able to tell you how/if you can reopen it. I would also recommend you seek out a local VSO to assist you with your claim. You seem to be needing some direction and VSO's are there to help you. If you are in doubt on who to use ask other vets in your area who they recommend. Thank you for your service,
